What do they usually do to girls in their physicals?

I haven't gone to the doctors for a physical in the longest & was just wondering what they do to you exactly. I've heard many things that bother me like the fact that they look at your vagina , not that I'm hiding anything it just makes me uncomftorable .. Can you refuse?
